expintx function — exponentially scaled expint

As commented in #244 (comment), it would be nice to have a function expintx(ν, z) = exp(z) * exp(ν, z), except computed by refactoring the expint implementation to factor out the multiplication by exp(-z), since otherwise a naive implementation of this function will suffer over/underflow.
