Use a Discord timestamp for entity mention dates

I imagine this would work best with a D or d format followed by an R format in parens, so what's currently Jan 21, 2025 would become 21/01/2025 (11 days ago) or January 21, 2025 (11 days ago) (both would differ based on the locale, of course).

In my opinion d looks pretty out of place, as opposed to a D.
