So fair I got:

kitten icat --use-window-size 1,1,500,500 --align left IMAGE
echo 'foo bar'

I tried to experiment with --align and --place but I couldn't understand how to achieve what I want with it.

Render the image with icat then move the cursor using an escape code and print the text, line by line.

I added echo -ne "\033[F\033[F\033[F\033[F" and (space) prefix at the start of every line, one big problem is when text is too long it wraps and ends up behind image. Like so:
