Error running tests on MacOS 12.01 | Safari 15.1

As a tester, I experience peculiar behavior when completing tests on macOS 12.01 | Safari 15.1

After completing a test and the Submit Results button is pressed the page returns an error state for all required fields. The status in the left nav tree shows the test as completed. If a user navigates away from the test to another test and then returns they will see the test as complete.

Steps to reproduce (not always seen on the first test)
Start testing
Fill out values for all fields in a test
Press the Submit Results button
If the test page returns correctly then proceed to the next test. If the page shows errors then the condition is reproduced.
