Based on feedback from @danielgblanco and @reese-lee I am removing the questions and adding metadata directly to each survey's README, rather than to a separate file. Please review. 🙏 I will create a separate PR to add issue template for surveys. FYI @avillela |

Hey, as a part of #85 I was reviewing current surveys. It was a bit difficult, because I had to dig through various GH issues to find the questionnaires in Google Forms. Since some of them were already closed, I had figure out the survey from the data from csv files.To make it easier I suggest to store this information directly in the repo.
I have a proposal ready. I would like to discuss following things about it:
