Hey, as a part of #85 I was reviewing current surveys. It was a bit difficult, because I had to dig through various GH issues to find the questionnaires in Google Forms. Since some of them were already closed, I had figure out the survey from the data from csv files.To make it easier I suggest to store this information directly in the repo.

I have a proposal ready. I would like to discuss following things about it:

Questions

  1. Do we need this at all?
  2. Do we want this info in a separate md file or should it live in README for each survey?
  3. Do we need metadata (links to issues, form, start date, end date etc)? If yes, what metadata?
  4. Do we need questions in md format?

Based on feedback from @danielgblanco and @reese-lee I am removing the questions and adding metadata directly to each survey's README, rather than to a separate file.

Please review. 🙏

I will create a separate PR to add issue template for surveys.
